Upon successful completion of the first two stages participants can move into our final stage; Continuing Care. This sees participants move off site either back to supportive family or into our supported transitional accommodation.
During this stage participants commence either work or study on a part time basis. We understand it can be stressful and overwhelming once participants leave the safety of a residential rehabilitation program which is why we have a dedicated team to support participants with this transition. A mental health outreach worker will assist with day to day support whist counsellors will offer continued care to both the participant and their families as needed.
We believe that the continued connection with Tenacious House greatly assists our participants in recovery and provides the support needed as they continue the journey of improved wellbeing.
