CONTINUING CARE

Continuing Care is the third phase of the Tenacious House program. Continuing Care sees participants move off site either back to supportive family or into our transitional accommodation which provides participants with a stable and affordable home to help transition back into the community.

During this phase participants will work to develop independence, commence either volunteering, study or work on a part time basis and take substantial steps towards realising their goals in achieving a healthy lifestyle. We understand it can be stressful and overwhelming once participants leave the safety of a residential rehabilitation program which is why we have a dedicated team to support participants with this transition.

Our outreach worker will assist the participants with day-to-day support, whilst our counselors offer continued care to both the participant and their families as needed. We believe that the continued connection with Tenacious House greatly assists our participants in recovery and provides the support needed as they continue the journey of improved wellbeing.
